Membership Form

You can enrol your organisation via this form. You will need to have a copy of your constitution in Word, RTF or PDF format so that you can upload it directly – if you require help with this or any other aspect of the form contact Sue Williams on 01352 744030.

Please read “Applying for Membership” information before completing the form.

Membership charges are:

  • Full membership (Voluntary /Community Groups): £15
  • Associate (Organisation): £25
  • Associate (Individual): £7.50

Once we have received your application we will send an invoice to the main contact address.

An asterisk (star) denotes a field that must be completed – otherwise you will will get an error message when you submit the form. Once the form has been submitted properly you will get a message telling you so. Phone (01352) 744030 if you have any problems.

I/We wish to apply for membership of Flintshire Local Voluntary Council (FLVC) for the year 1 April 2011 to 31 March 2012 or I/We wish to update the information that FLVC holds on our organisation.

  • This information will be held by FLVC on computer and those details with an asterisk published on the Internet when appropriate. The information will be used internally by FLVC in the administration of its membership, in the answering of queries and be made available to those agencies working in partnership with FLVC, where appropriate.
